Our New Patient Packet is available for download. You can save time by filling this out before your first appointment. Please do not sign until you are in our office, so one of our staff members can witness it as required. Please bring a non-expired government ID and insurance card(s).
Please note if you have been seen here before but it was over a year ago, you will have to complete new patient paperwork but you will be considered a returning patient.
To schedule an appointment, call our office at (910) 347-3010 and select option 1. The caller should be the patient, for minors: biological parent or legal guardian of the patient (must provide legal documentation for example a court order or Power of Attorney). Please be prepared to give full name, date of birth, address, phone number and insurance information - including policy holder name, date of birth, social security number and address.
For the minor patient: the biological parent or legal guardian (with legal documentation) must be present at the initial appointment.
Patient paperwork may be completed in the office prior to the appointment time. It may also be downloaded from this page for your convenience, we ask that you do not sign or date the paperwork until you are in our office and it is reviewed with our staff who will witness your signature.
Each scheduled appointment is reserved exclusively for you. If you cannot keep your appointment, please call and let us know, you may leave a voicemail outside of business hours. We require at least a 24-business-hour notice to cancel or reschedule any appointment. This allows us some time to try to offer another patient the allotted time.
Please arrive 30 minutes prior to your first appointment to allow time to go over the required paperwork.
For subsequent appointments, we ask that arrive approximately 15 minutes prior to your appointment to complete the check-in process.
What should I bring to my first appointment?
Bring your health insurance card (s), a list of current medications (and dosage), and a non-expired government photo identification (driver’s license, military ID card (which also acts as your health insurance card), and payment, we accept cash, check, money orders, Discover, Mastercard and Visa.
