Patient Insurance Information
Carolina Psychological Health Services is in-network with Blue Cross Blue Shield, MedCost, Medicare, TRICARE, United Healthcare, and VA Community Care. We also accept other insurances that may be out of network. Please contact us to determine if we accept your insurance. Our staff is here to answer any questions regarding your coverage and to help determine your estimated cost of treatment.
New Patient Information
To schedule an appointment, call our office and speak with a receptionist. For minors, the caller should be the patient’s biological parent or legal guardian (must provide legal documentation, for example, a court order or Power of Attorney). Please be prepared to give full name, date of birth, address, phone number, and insurance information, including policyholder name, date of birth, social security number, and address. For the minor patient: the biological parent or legal guardian (with legal documentation) must be present at the initial appointment. Patient paperwork may be completed online via the patient portal or in the office before the day of the appointment. Each scheduled appointment is reserved exclusively for you. If you cannot keep your appointment, please call and let us know. We require at least a 24-business-hour notice to cancel or reschedule any appointment.
Expectations of Your First Appointment
Please arrive 30 minutes before your first appointment to allow time to go over the required paperwork. For subsequent appointments, we ask that you arrive approximately 15 minutes before your appointment to complete the check-in process. Your first appointment is critical as we get to know one another on a deeper level to address your needs better. You need to bring your health insurance card(s), a list of current medications (and dosage), and a non-expired government photo identification (driver’s license, military I.D. card), and payment. We accept cash, check, money orders, Discover, Mastercard and Visa. Contact us with any further questions.
*Our office is currently enforcing additional precautions due to COVID-19 to protect the health and safety of our patients, staff and loved ones. Please be sure to wear an appropriate face mask to cover your mouth and nose when entering the building whether you have or have not received your COVID vaccines. If you do not have one, please inform the receptionist as you check in and one will be provided for you.
